• Discuss with your loan officer which items will be necessary.
• Make CLEAN & LEGIBLE copies of the applicable items.
• Fill out, sign and date the REQUIRED DISCLOSURES.

• Fill out, sign and date the LOAN APPLICATION.
• Unmarried co-borrowers must complete SEPARATE loan application forms.

Assemble your LOAN APPLICATION, REQUIRED DISCLOSURES, and CLEAN LEGIBLE copies of all applicable items from the DOCUMENTAION CHECKLIST.
